What is the Baker classification scale?
The Baker classification scale is a four-grade system used to measure the severity of capsular contracture, a condition where scar tissue hardens around a breast implant.
The Baker scale rates capsular contracture, a post-operative complication where the natural scar capsule surrounding a breast implant tightens and hardens over time. Surgeons use this four-point grading system to assess firmness and deformity during physical examination.
The grades are:
- Grade I: Breast feels as soft as an unaugmented breast with no visible deformity.
- Grade II: Breast is slightly firm but appears normal with no visible changes.
- Grade III: Breast is firm with visible signs of contracture, such as distortion or fullness in the upper pole.
- Grade IV: Breast is hard with significant pain and visible deformity affecting implant position and shape.
Capsular contracture affects a small percentage of breast augmentation patients. Grade I and II cases are usually asymptomatic and do not require intervention. Grade III and IV cases may cause pain, asymmetry, or dissatisfaction with appearance and often lead patients to seek corrective surgery.
Plastic surgeons use the Baker scale to document findings during routine follow-ups, communicate severity with patients, and guide treatment decisions. When a patient feels their breast implant becoming progressively firmer or uncomfortable, the Baker assessment helps determine whether observation, massage, or surgical intervention is appropriate.
