What is the SMAS in facial plastic surgery?

The SMAS is the layer of muscle and connective tissue beneath the facial skin that surgeons lift and tighten during facelift surgery to restore definition and address descent caused by aging.

The superficial musculoaponeurotic system, or SMAS, is a continuous layer of muscle, fascia, and connective tissue that lies directly beneath the skin of the face and neck. It extends from the scalp down to the collarbone and encompasses the muscles responsible for facial expression. In younger faces, this layer stays taut and supports skin position. Over time, gravity and loss of elasticity cause both the skin and SMAS to descend, creating jowls, marionette lines, and loss of jawline definition.

During a facelift (rhytidectomy), surgeons access and lift the SMAS to restore it to a higher, more youthful position. By tightening this deeper structural layer, rather than simply pulling surface skin, the procedure achieves longer-lasting results and more natural-looking contours. The technique allows surgeons to address sagging not just at the surface but at the source of facial descent. Different facial plastic surgeons may use variations in SMAS technique, from traditional plication (folding and suturing) to extended SMAS approaches, depending on patient anatomy and desired outcomes.

Understanding SMAS anatomy is fundamental to modern facelift surgery because lifting only the skin without supporting the deeper structures tends to produce a tight, unnatural appearance that does not last. Working at the SMAS level allows surgeons to redistribute facial volume more effectively and achieve results that look refreshed rather than overdone.
